Register on or before your first visit!

You can always register in person at your local center. If you like, you can print our Registration Form and bring it with you already filled out. There is a one time Family Registration fee. See details.

Registration Form

We require family information, 2 emergency contacts with their address and phone number and specific information about your child's health.

Please note:

• Print neatly.
• All fields with an * are required.
• Your registration remains active for 12 months from your last visit date. After that, it is deleted and you need to re-register.

If your young child looks younger than the center's minimum age, please bring proof of age (birth certificate, immunization record or anything with a birth date).
